It is possible for patients to self refer for NHS Physiotherapy Treatment. This means that you do not need to see your GP to be referred to an NHS Physiotherapist.
If you have a condition that you think may benefit from a course of physiotherapy, you can complete a Self Referral form available from your GP Surgery. This is should then be forwarded directly to the Physiotherapy Patient Contact Centre who will liaise directly with you about whether physiotherapy is a suitable option for you and to arrange how and when treatment can be carried out.
There are some exceptions to self-referral by patients which are detailed below:
- Under 16’s for patients who are under 16 years of age a GP referral is required
- Complex Medical History for patients with a heart condition, who have been treated for cancer or any other complex medical problem a GP referral is required
- Language Issues for patients who do not speak English to a level where they can complete a self referral with all the information needed a GP referral is required
- Foot Problems any problems relating to the feet need to be dealt with by a podiatrist
Once completed the form should be either posted to the following address:
Patient Contact Centre, Room F20/F21, First Floor, Highfield Clinical Care, Cliftonville Road, Northampton, NN1 5BU
or e-mailed to [email protected]
Alternatively you can self refer by telephone on 0330 555 6789.
